Internal Medicine Rotation I and II are two of the
required rotations for the PGY1 residency. Each
rotation is a 1 month block of time occurring nonconsecutively during the 12 month residency
program. The resident will be an active member of
the SRAHEC internal medicine team and participate in
all team activities. Patients are distributed
throughout the hospital floors at Cape Fear Valley
Health System. Responsibilities include
provision of pharmaceutical care by reviewing each
patient profile for appropriateness of drug therapy,
performing pharmacokinetic dosing and monitoring on
all therapeutically monitored drugs (TDM), obtaining
medication histories, performing discharge
medication counseling, providing drug information to
team, training and educating medical residents on
the appropriate use of medications, and precepting
fourth year pharmacy students.
Resident Learning
Activities
-
Actively participate in work rounds with
medicine team daily.
-
Actively participate in attending rounds with
medicine team daily.
-
Present
patients to preceptor on a daily basis and
formally present two patient cases during each
month to the pharmacy staff or internal medicine
pharmacy team.

Critically appraise and present one journal club article to the pharmacy
staff or internal medicine pharmacy team.
-
Attend SRAHEC noon conferences every Tuesday and
Thursday at 1200 in the Family Medicine
Classroom unless excused by preceptor.

Document or update allergies, height and weight
on all patients as needed in the pharmacy
computer system.

Perform
pharmacokinetic monitoring on all patients on
medicine team.
-
Document
interventions/recommendations in the patient’s
medical record as needed.

Give a minimum of 1 inservice to the medicine
team.

Meet with your
preceptor to discuss patient care and various
disease states (e.g. Asthma/COPD, renal failure,
liver failure, diabetes mellitus, pneumonia,
AIDS, pancreatitis, meningitis, DVT/PE, etc.).

Complete the One Minute Preceptor Course
available online and review concepts with
preceptor

Attend scheduled educational sessions for PY4
pharmacy students as planned during the month
-
Prepare and teach pharmacokinetic dosing and
monitoring of vancomycin, aminoglycosides,
heparin, and phenytoin to PY4 pharmacy students.

Precept and
serve as role model for fourth year pharmacy
students.
