Nephrology

The Nephrology rotation is one of the elective options that may be selected as part of the PGYI residency. The rotation is a month-long study that will be appropriately scheduled within the 12 month residency program. The resident will divide their time between the Nephrology and Hemodialysis units at Cape Fear Valley Health System and one of the Health System-affiliated outpatient dialysis clinics in the county. The resident will round daily with the on-call nephrologist and actively participate in therapeutic decision making for patients on the nephrology service.  The resident will review patient profiles for appropriateness of drug dosing and will become proficient in adjusting medication regimens in the renal population.  In addition, the resident will provide pharmaceutical care by obtaining accurate medication histories, addressing drug information inquiries by physicians and nurses, attending interdisciplinary rounds, providing pertinent educational inservices for unit staff, and offering discharge medication counseling to patients and their caregivers.

Resident Learning Activities

  1. Actively participate in daily patient rounds with the on-call nephrologist.
  2. Actively participate in weekly interdisciplinary rounds with the nephrology unit staff.
  3. Observe and understand the daily routine in the inpatient hemodialysis unit.
  4. Visit outpatient hemodialysis clinic on a weekly basis and observe the daily routine in that patient care setting.
  5. Discover and devise opportunities for pharmaceutical care in the outpatient hemodialysis clinic.
  6. Implement a mini-project that demonstrates the impact of pharmaceutical care in the outpatient hemodialysis setting.
  7. Provide bi-weekly educational inservices to the nephrology unit staff on a relevant topic.
  8. Present one journal club article to the pharmacy staff that is specific to pharmaceutical care in the renal population.
  9. Present a patient case to your preceptor each week.
  10. Obtain accurate medication histories and perform allergy assessments on a daily basis.
  11. Provide discharge counseling to patients when appropriate.
  12. Perform renal dose adjustments and pharmacokinetic monitoring on all renal and hemodialysis patients.
  13. Become familiar with colony stimulating factors (Epogen®, Procrit®, Aranesp®, etc.) and develop an understanding of the use and dosing of these agents in the renal population.
  14. Review patient profiles daily for appropriateness of drug therapy, renal dose adjustments, and make recommendations to physicians when necessary.
  15. Develop problems lists and define therapeutic goals.
  16. Document interventions/recommendations in the patient’s medical record when appropriate.
