Opportunity for a practice coach/consultant to provide “hands-on” transformation services in primary care and specialty practice sites in a nine-county region. Position will provide project management for individual practices involved in a variety of grant-funded quality improvement and health IT initiatives all leading to improved clinical outcomes. Bachelor’s degree in health related field plus two years experience in the healthcare setting. Strong health IT skills a plus. Extensive regional travel. Competitive salary and excellent benefits.
