Southern Regional AHEC is recruiting for a Medical Case Management Coordinator to provide medical case management services to help eligible individuals enroll in the Ryan White HIV Program. Responsible for performing initial assessments, developing care plans and providing ongoing support and monitoring. Must have strong organizational, interpersonal and customer service skills. Bachelor’s degree in Social Work or human services field required. As a 501(c)(3) non-profit organization, employees may be eligible for the federal Public Service Loan Forgiveness (PSLF) Program.
